How to be a donor:

--Sign and carry a donor card or sign the back of your Tennessee driver's license.
--Sign your driver's license or application.
--Tell your family you would like to be an organ and tissue donor when you die.
--In the fall of 2007, Tenneseeans will be able to register on-line.

Indicating your wish to be a donor signifies your commitment to renewing the life and health of others in need. In the event of your death, your family will be informed of your decision to be an organ and tissue donor and involved in the donation process. Now is the time to sign up and talk to your family about donating life. You have the chance to make such a difference to the world.

--We suggest you have two members of your family sign their names in the spaces provided. This ensures they know your wishes.
--Carry this card in your purse or wallet where it can be found.
