What It Means To Be A Donor
Organ and tissue donation is a way to share life with others. By putting
your name on the Donate Life Tennessee Organ and Tissue Donor Registry, you consent
to having your organs and tissue made available for transplantation upon your death.
• Examples of organs that are needed for life saving transplants include heart, liver, pancreas,
kidneys, lungs and small bowel.
• Examples of tissues that could save or improve someone's life include eyes/corneas,
heart valves, bones and skin grafts.
Registering As a Donor
All information submitted will be used only for official Registry business and will
be kept completely confidential. Your information is only shared with organ and
tissue recovery agencies that are authorized by Donate Life Tennessee to verify
this information in the case of a death of a registered donor. Hospital personnel
do NOT have access to the Donor Registry. We will not share, sell or otherwise compromise

Specifying Your Donation
If you are between 13 and 17 years old AND have a state ID card or Driver's License/Learner's
Permit, you can join the Donate Life Tennessee organ and tissue donor registry.
However, please be aware that your parents will make the final decision about organ and tissue donation at
the appropriate time.
It is possible to specify how your would like your organ and/or tissue donation to be handled:
You can decide which specific organs or tissue you would like to donate. These may include
heart, lungs, liver, kidneys, pancreas, small bowels, eyes/corneas, skin grafts, heart valves, bones and soft tissue
(i.e., tendons) or veins and arteries.
You can decide to limit your donation for domestic use only.
You can decide to limit your donation for use only by nonprofit organizations.
You can decide to donate your organs and/or tissue for education or research purposes, if it is determined they cannot
ne used for transplants.
If you check "YES" below you will be given those options. If you do NOT check "YES," you are agreeing to donate all
organs, eyes and tissue for domestic and/or international use, and for research and/or educational purposes.

Uniform Anatomical Gift Act. A document of gift, not revoked
